QA Surveys

Navigate to the Surveys section using the Company Data menu (Company Data > Survey Data > Surveys).

To return to the previous page, click the  Back button. To print the module, click the   Print button.


To add a new survey, click on the  Add button in the blue module bar.
  1. To edit an existing survey, click on the  edit pencil next to the survey you wish to edit.
  2. The Edit Survey Data screen will appear.


  1. Enter/edit the name of the survey.
  2. Enter Script or QA for the survey type. Note: QA survey types display an icon on the order detail screen once a sale has been made, and script surveys display when the call is entered into the database.
  3. You can use the different options to customize the survey header.  For information on the options, click the  Help icon. This will display a list of options and what each one does.
  4. Check HTML Mode if you want to enter in HTML code or copy from another source. 
  5. To add a survey question, click on the  Add link.  To edit an existing question, click the  edit button.
  6. A Survey Question Data screen will appear.
  7. To delete the question, click the   delete button.
  8. To see the values contained in a list response, hover over the List Values button.


  1. To edit the values of a list response, click the List Values   button.
  2. A List Values screen will appear.
  • To save changes to the survey, click the  Save button. This will save the current changes and close the Edit Survey Data window.
  • To cancel any of the current changes you have made, click the   Cancel button. This will close the Edit Survey Data window without saving any changes.
  • To delete the survey, click the Delete button. This will prompt you to make sure you want to delete the survey. Clicking yes will delete the survey and close the Edit Survey Data window.

    Note: You cannot delete when you are trying to add a new survey.
  • To print the current Survey Data window, click the   Print button.


Survey Question Data

The Survey Question Data screen lets you specify response options for questions pertaining to a given survey. Navigate to Company Data > Survey Data > Survey Questions. 


Note:  The Survey list is only active when adding a new question from the Survey Questions screen. 

  1. In Short Display Text, enter a brief description of the question.
  2. Enter the display order for this question in the survey.
  3. Click on the hyperlink number displayed in Rules , if you want to add, edit, or delete a rule pertaining to this question.
  4. Check Active to activate/deactivate the question.
  5. Check Required if this is a mandatory question.
  6. Check Allow Comments to allow the customer/representative to provide additional feedback.
  7. Check Recording Checkpoint to add a field used to display the time of the recording for the question. Note: The recording checkpoint will only appear for QA surveys.
  8. Enter the Response Type. This can be either Alpha, Date, Numeric, Boolean (Yes/No), or List . A selection box will appear in the upper right corner of the screen depending on the response type with the criteria that you can choose for values.
  9. Enter the Question Text. This is the text that will appear in the Text section of the survey.
  10. You can use the different options to customize the survey header. For information on the options, click the Help icon. This will display a list of options and what each one does.
  11. Check HTML Mode if you want to enter in HTML code or copy from another source.
  • To save changes to the survey question, click the  Save button. This will save the current changes and close the Survey Question Data window.
  • To cancel any of the current changes you have made, click the  Cancel button. This will close the Survey Question Data window without saving any changes.
  • To delete the survey question, click the Delete button. This will prompt you to make sure you want to delete the survey question. Clicking yes will delete the survey question and close the Survey Question Data window.

  • To print the current Survey Question Data window, click the  Print button.


List Values

The List Values Screen shows selection options for survey questions with various list response types.




  • To change the list type, select the Render List As drop-down menu.  List options include: Drop Down List, Radio Button List, Dual Multi-Select list, and Check Box list.
  • To add a survey list option, click on the  Add link.  To edit an existing option, click the  edit button. To delete the option, click the   delete button.
  • To cancel editing, click the  cancel icon. This will cancel any changes made to that user or group.
  • Enter the display order regarding where in the list the value should appear.
  • To save the changes, click the  save icon.
    • To save changes to the list, click the  Save button. This will save the current changes and close the List Values window.
    • To cancel any of the current changes you have made, click the  Cancel button. This will close the List Values window without saving any changes.
