Please fill out and return the following form so that our IT team can complete your
request.

  1. Today’s Date
  2. Company Name
  3. Your Name
  4. Email Address
  5. Type of Service: SMTP
  6. SMTP Server FQHN (Fully Qualified Host Name, e.g.: smtp.server.org)
  7. SMTP Server IP (e.g.: 192.168.0.45)
  8. SMTP Server Port: 25
  9. Notes
Please include multiple IP addresses if applicable (in cases of redundant connections, load balancing, etc.)
